Student-Athlete Administrative and Health Forms

Welcome to Hudson Valley Community College Athletics!

Hudson Valley Community College Department of Athletics utilizes PRIVIT to collect all student-athlete eligibility forms. All student-athletes must register and create a PRIVIT account through the HVCC Privit portal and complete all of the required documents and trainings in order to be cleared for athletics participation.

MEDICAL FORMS: Please note student-athletes must download and print the HVCC Physical and Meningitis Form. These forms need to be filled out by your physician and uploaded into your PRIVIT portal. All physicals are reviewed by health services and approved by the college physician in the order in which it is received.

*A Student-athletes eligibility cannot be determined without a copy of the transcripts from each institution of higher education attended. These transcripts are required regardless of athletic participation at any institution. Please schedule an appointment with the Director of Athletics (athletics@hvcc.edu) to review your eligibility prior to enrollment.

Important Note: Please be aware that when submitting paperwork, you are verifying that all information you provide is accurate and truthful. The information will be maintained by Hudson Valley Community College and the NJCAA as requested. Any individuals submitting false or inaccurate information will be subject to disciplinary action including, but not limited to, dismissal from the respective athletic program(s) and legal action. Per Article V, Section 1.F of the NJCAA by-laws, any student-athlete who falsifies any academic and/or athletic participation record shall be ineligible for further competition in an NJCAA member college at any time. The scope and complexity of NJCAA legislation precludes complete coverage on this website. We suggest, therefore, that you forward additional questions to the Director of Athletics.
