URGENT: All Vikings football parents, staff and fans planning to attend this weekend's Vikings football game at Navy Prep in Newport, Rhode Island MUST complete the attached form and return A.S.A.P. to m.muehling@hvcc.edu.
All fans, parents, HVCC support staff arriving by POV will need to be vetted and placed on the access list for Saturday 03 Nov 2018. Each person must provide a GOV ID for base entry.
How to be vetted:

Fill in block 1 through 13
Block 14 - security only requires a social security number, do not provide drivers licence # or passport #, security will not use these to vett you.
Check box 14 and write your number in block 15.
Fill in block 20 through 24 (include contact phone number)
Fill in block 26 and 27, HVCC students should write student then give HVCC address

Check yes or no in block 29.
Print form - each person must initial block 29 and 30, then date and sign form.
All forms need to be neatly printed or security will reject. Security must be able to read the the complete form.
